Warning/Waiver, Release of Liability / Agreement to Hold Harmless

GENERAL LIABILITY RELEASE

I acknowledge that there are dangers of injury in martial arts. I agree that prior to every session I
will inspect/observe the training environment. If I believe anything is unsafe or beyond my
capability, I will immediately advise the members and elect not to participate and/or excuse
myself from the premises. I acknowledge and fully understand that I will be engaged in a
contact sport and combat art that might result in serious injury, including permanent disability or
death, and severe social and economic losses due not only to my own actions, inactions or
negligence, but also to the actions, inactions, or negligence of others, the rules (written or stated)
or techniques, or conditions of the premise or of the equipment used. I further acknowledge that
there may be other risks not known to me or not reasonably foreseeable from time to time. I
assume all risks and accept personal responsibility for the damages resulting from injury
including but not limited to permanent disability, or death.
I release, indemnify and hold harmless personally, professionally, Jiu-Jitsu Syndicate, LLC,
partners, successors, heirs, staff, coaches, and members or any individual providing instruction,
their respective administrators, directors, agents, and other employees or volunteers of the
organization, event officials, medical personnel, other participants, their parents, guardians,
supervisors and coaches, sponsoring agencies, sponsors, advertisers, and if applicable, owners,
lessors, and lessees of premises used in conducting sessions, all of whom are hereinafter referred
to as indemnified and held harmless from any and all claims, demands, losses, or damages
including but not limited to disability and death, damage and loss of personal property, caused
by, alleged to be caused in whole or in part by the negligence of the releasees or otherwise to the
fullest extent permitted by law.
I HAVE READ THE ABOVE WARNING, WAIVER, AND RELEASE, UNDERSTAND THAT
I GIVE UP SUBSTANCIAL RIGHTS BY SIGNING IT, AND KNOWING THIS, SIGN IT
VOLUNTARILY. I AGREE TO PARTICIPATE KNOWING THE RISKS AND CONDITIONS
INVOLVED AND DO SO ENTIRELY OF MY OWN FREE WILL. I AFFIRM THAT I AM AT
LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED
THE REQUIRED CONSENT OF MY PARENTS/GUARDIAN AS EVIDENCED BY THEIR
SIGNATURE BELOW.
This is to certify that I, as parent/guardian with legal responsibility for this participant, do
consent to and agree to his/her release, as provided above, of all the Releasees, and, for myself,
my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the
Releasees from any liabilities incident to my minor children involvement or participation in these
programs as provided above, even if arising from their negligence, to the fullest extent by law. I
have instructed the minor participant as to the above warning and conditions and their
ramifications. I fully understand the above and will abide by its word and its intent.

PHOTO AND VIDEO RELEASE

I hereby grant Jiu-Jitsu Syndicate, LLC permission to use my likeness in a photograph, video, or
other digital media (“photo”) in any and all of its publications, including web-based publications,
without payment or other consideration.
I understand and agree that all photos will become the property of the Jiu-Jitsu Syndicate, LLC
and will not be returned.
I hereby irrevocably authorize Jiu-Jitsu Syndicate, LLC to edit, alter, copy, exhibit, publish, or
distribute these photos for any lawful purpose. In addition, I waive any right to inspect or
approve the finished product wherein my likeness appears. Additionally, I waive any right to
royalties or other compensation arising or related to the use of the photo.
I hereby hold harmless, release, and forever discharge Jiu-Jitsu Syndicate, LLC from all claims,
demands, and causes of action which I, my heirs, representatives, executors, administrators, or
any other persons acting on my behalf or on behalf of my estate have or may have by reason of
this authorization.
I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I
AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE
OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS
EVIDENCED BY THEIR SIGNATURES BELOW. I ACCEPT:
